To sum up

User1 sends email to [EMAIL PROTECTED]: attachment not recieved
User2 sends email to [EMAIL PROTECTED]: attachment received.

Now, here's some random senerios.
1. Recipient1 is blocking by Attachment type? (I know we do).
2. User1 is infected by a virus and the email is being striped with no end
user notification.
3. User1 is not really sending an attachment, merely thinks he/she is.

Based on your info, I would ignore random senerio 1 and look at 2-3.
Try with the same users from a known good system that works, then try with
the same users from the 'known bad' system

No , the recipient does not reside in our server. 
Yes , we do have Trend anti-virus. 

What I'm thinking of is that it is not consistent.
One user would send a message w/ attachment to a particular recipient that
is outside the server. 
Recipient would receive message without atachment.
Another user will send the same attachment to the same recipient and the
recipient will receive the attachment. Why?
