Do you have an open ticket with MS, because it seems like it is not resolved.
Did they mention to you the mailbox does not get created right away? Not until the address is updated in AD by the recipient update service, then the mailbox receives a message or someone logs into it. As for users disappearing from the GAL, I'd ask what changed and certainly a call to Microsoft is worth it.
