Any time the server is accessed, you need a CAL. If every device being used to access the server already has a CAL, that satisfies the requirement. If some devices have CALs and some do not, you need to get CALs for the additional devices. Device CALs are assigned to specific devices. If arbitrary computers are being used (e.g., access from the public Internet), you'll need to go with per-user CALs.
