Just to make this more weird, now I'm not getting a defer, I'm getting a 550 5.1.1 RESOLVER.ADR.RecipNotFound. This is only when I'm trying to send a message from the outside to a public folder that is only in Exchange 2003 using one of my 2010 CAS Arrays. If I send the same message to a different CAS Array it goes through fine. Both CAS Arrays go to the same DAG but they deliver to different AD Sites.
