Just a WAG: check the permissions on the OAB web.config file and make sure that the authenticated users group has read and execute permissions. For some reason about 3 weeks after we started using Exchange 2010 we had login dialog boxes start appearing in Outlook and the GAL was not updating in Outlook and this was the culprit. The web.config file is in program files\microsoft\exchange server\v14\clientaccess\oab\web.cong
