My first thought is incorrect formatting. We see this quite a bit when the disks are not formatted correctly. They should be formatted with 64k, not the default size. I guess I should be asking if this is a clustered environment. If you do a google or bing on 1022 errors, you will come up with the exchange blog and entries my Mike Lagasse, he's a very sharp guy and extremely knowledgeable on the subject +. You can read up on the problem and some fixes. Formatting is the number1 cause from my perspective.
