I'm using debian's version 4.69-6 of exim4-daemon-heavy. This is very much an intermittent thing, so it's going to be hard to reproduce... however, here (reject.txt) is a message being rejected immediately. (Email addresses have been changed to protect the guilty^Winnocent, and this mail had a bunch of other recipients which I've also elided from the log).
