Configuration options are inline with Exim. In practice, as previously discussed in this bug, most people will have ClamAV up-to-date and won't need to worry about this. This is dead code for most people and there's no need for it to remain in. I expect that there will be some polls and at some point in the next couple of years we'll be confident that ClamAV < 0.95 truly is dead, and remove the old logic entirely. For the hypothetical OS distributions doing such things, either they are or they are not. Mostly, when they update ClamAV, they can change the build option for Exim too. They can build Exim with a dependency on ClamAV >= 0.95, or a dependency on ClamAV < 0.95.
