The trouble is they were using "per_rcpt", which doesn't make sense in the not_smtp acl because it is keyed on "$local_part@$domain", which in the not_smtp acl is simply "@" because it is run once per message rather than once per recipient so $local_part and $domain are both blank. So they were inadvertently ratelimiting on total number of messages rather than total messages per recipient.
