Use of _DH_H_ is deliberate as can be seen from the comments.  While we could
invent yet another preprocessor identifier for a guard to explicitly lock
out the system db.h inclusion I'm not convinced it is worthwhile.

__SRS_H__ is not reserved as it does not begin with an underbar followed by
a letter.  Thus it is legitimate for application source code use.
