Have you checked that you don't have lots of users who are on the same
mailing list? We had some problems here when a mail server in the
university started bouncing email that had more than a few tens of
recipients, which triggered quite frequently because many users on that
mail server were on big mailing lists.

If you are rate-limiting, the ratelimit ACL condition has both per_mail
and per_rcpt options.
