This is the basics of what you need to do for sending to an alternate
port on a remote host (smarthost)

Router:

send_to_smarthost:
  driver = manualroute
  transport = remote_smtp_relay
  route_list = *

Transport:

remote_smtp_relay:
  driver = smtp
  delay_after_cutoff = false
  no_retry_include_ip_address
  port = 26
