You could try upgrading (4.89 is in backports).
Other than that, is /var/spool/exim4 on a normal local filesystem?
It turned out that the exim_tidydb and dumpdb were leftovers from
compiling them myselves while there were leftovers from libdb in
/usr/local/
Recompile/Reinstall was fixing it after cleaning up /usr/local/
