I have now identified bug 2025, fixed in Exim 4.89 as the probable cause of this issue.

To summarise, when I was testing with SWAKS, this substitution of an equal sign for a space occurred. If I cut and pasted the MAIL and RCPT lines reported by SWAKS into a telnet session, no substitution occurred. When I upgraded the Exim binary from 4.86 to 4.89 then no substitution occurred in either case. We have no explanation, but upgrading seems to offer a solution. Thanks Heiko for your assistance.
