As others have said, you are more likely to get a useful response from the
Debian people.

One possiblity is that SELinux contexts are involved
        man -k selinux_restorecon
or
        man -k restorecon
may give you a hint as to where to start.

I already done that, and exim4 is running as the right user.

periodically, are changing the owner of the /var/spool/exim directory to
Debian-exim, causing lots of permission errors.

If you do not want to use the Debian-exim user, you may override the
compiled user by setting:

exim_user       exim

exim_group    exim
