Apart from this, you can also ask the ASL for a special declaration (I don't remember the name), which provides you some coverage in country like Russia - where there is no mutual agreement for health insurance. Basically it says that if you undertake remarkable expenses, you can ask our italian national system for compensation, and they will evaluate case by case So, standard insurance + this declaration: it's more than enough. In addition, should you get seriuosly ill, I'm sure you would go somewhere else for medical assistance (probably back to Italy, or to Europe...)
