Or the card perhaps is not seated in the slot.

I had this happen to me with my SB128 on my current machine when I built it.

It drove me nuts for a couple of days.. :)

The card was in and working.. I added something and somehow dislodged the card out of 
the
pci slot
away from the back. Just enough to cause problems, not enough to obviously notice it.
