I'm not familiar with that ADSL modem, but here are some ideas:

- see www.annexia.org, there is a useful ADSL howto for Linux there (by
another Richard Jones!)
- search google.com for 'us west adsl linux'
- search deja.com/usenet for the same
- if no luck, post on a suitable linux+networking forum via deja.com

Most ADSL kit runs either PPP over ATM or PPP over ethernet.  So have a
look for PPPoA as well.

You will also need to have a look for specific drivers for your modem -
at least it's a PCI model. Here in the UK our much loved national telco,
BT, has chosen a USB-connected ADSL modem - amazingly enough, there is
already a Linux driver for this only a few months after first release,
so you may be in luck!

One other idea - make sure you post in any US West support newsgroups,
other local users may have solved this problem already.
