I just finished fixing a similar problem with my voodoo3.  From your error 
message, it looks like the glx module isn't being loaded when X starts.

First, you need to load the kernel module tdfx (requires the agpgart module 
if you have an AGP card)

Then make sure you have these lines
load "glx"
load "dri"
load "tdfx_dri"
load "glide_drv"
in the module section of /etc/X11/XF86Config-4

Also, 3D acceleration only works in 16-bit mode on a voodoo3.  You can set 
this in X11/XF86Config by changing the DefaultColorDepth line from 32 or 24 
to 16.
