Maybe you not run lilo, and if you try it, check that have a line like this
initrd=/boot/initrd-2.2.17-21mdk.img

in the paragraph of you kernel image, here is an example, that I found in my
/etc/lilo.conf
image=/boot/vmlinuz
        label=old_linux
        root=/dev/hda9
        initrd=/boot/initrd-2.2.17-21mdk.img
        append=" ide1=autotune ide0=autotune"
        read-only
