I don't have any info on the extra mouse buttons.  (personally I find 3
buttons and a wheel to be my limit.)  But I did just find out how to
make my wheel mouse work with a kvm switch.

1.  During the install have the mouse directly attached to the computer.
2.  Boot into X for all the users with the wheel mouse directly
attached. 3.  Shut down the box.  Move the mouse to the kvm switch and
attach all cables.  Set /etc/inittab to run level 3. 4.  boot the box
and do a /etc/init.d/gpm restart once or twice to get the mouse
working.(I also found that switching comps once or twice helped too.) 5.
 Telinit 5 and the mouse works.  reset /etc/inittab to run level 5.

It has so far worked on reboots without a qualm.
