Have you looked at the docs:
file:///usr/share/doc/HOWTO/HTML/en/Xinerama-HOWTO.html

Maybe you need to edit your /etc/X11/xdm/Xservers file as explained in 
the HOWTO in order to start the xinerama extensions.
