Could it be a plug failure?  While I have never heard of the central
electrode breaking internally and using gravity to come down to fuse with
the protruding electrode....that doesn't mean it can't happen.

The ground moved iniward to fuse with the positive Chuck, but it doesn't
look like like anything has been mashed or smashed.  It's just a really
nicely fused thogether with no sign of anything being hit. There is a just a
graceful smooth arc downward to the positive.
