There is certainly more 'meat' to this story. *Why* would they refuse? For example, were they threatened with a cessation of or reduction of US aid if they went for it? That has certainly happened in the past, with the US threatening to cancel aid to a country if it allowed free abortions or dispensing of birth control products.
