It's possible your form is throwing an error, but FarCry is suppressing it. I'm not sure exactly what the setup is here, but you might be able to get some information by adding bdebug=1 (showdebug? can't remember) or looking through the source code of the page - sometimes FarCry adds a comment to the html code when it has an error.

This may seem very elementary however I am having problems getting any
includes working.  I wrote a simple "include" displaying a form that a
user can submit.  The form does not display.  I have tried both
"display methods".

I have been able to properly select the actual include file I created
from the "include" drop down list also.

I am not understanding why they form is not displaying.  I have gone
through all the documentation I have been able to find on FarCry and
followed the steps for creating an include like the examples given in
the docs.

Thank you for your assistance,

Darren
