I guess in my case, both! Perhaps that's why it wasn't working for me. We are running CF in the multi-server configuration and also have more than one site running in IIS. We installed the filter globally but only enabled it on the specific site we needed it on. Like your situation, it all seemed to be configured correctly, it just didn't work.
