I've occasionally had trouble with the tree when the objectid has been added to the tree, but the object hasn't been successfully created. i.e. the tree refers to objects that don't exist. I assume a similar situation could come up when deleting objects. Rebuilding the tree didn't help, I had to manually go in and update the table.
