I looked into this a bit today and from the looks of it, you are right. If there is no parent approvers do not get selected. It looks like a potential feature request. However, I'm pretty sure that if you absolutely require approval, you could add the type directly in the site tree. You can add images, files, etc that way as well. Then a parent should exist because the system must keep track of what node an object belongs in. When you just add something to a media library, it doesn't seem like it is inserted in the nested_tree_objects table. You might want to add this to the bug tracker to see if it gets changed (http://bugs.farcrycms.org:8080).
