While logged in, view a page on the site. Using the context menu go  
into design mode.  Now add a rle on a page and choose the feedback  
rule. Turn off design mode and you'll see the form on the page of  
choice.

Keep in mind that the feedback rule (like many of the other features  
in the CMS plugin) is a sample app.   It's likely not going to give  
you 100% exactly what you want.  But it is a great starting point to  
look at the code and cusomize it for your project (unless of course it  
is exactly what you want, in which case you are good to go :) ).
