You could override (or add another) the insertHTML template for dmImage. By
default it just has the IMG tag, but you could easily add other HTML.

If you need to be able to add caption in tinyMCE I don't know what the
options are. You could try the tinyMCE community / forums - FarCry makes it
easy to edit the tinyMCE config if that's what it takes.
