Geoff, it is out of the box CFMX6.1 PRO with no hotfixes (I think it is jrun updater 2, but as it comes out of the 6.1CFMX install).
Almost 100% going to be the fact you haven't patched the SQL server JDBC driver -- there is a known issue with this driver that can cause intermittent failure of this kind. You need to apply the updaters :)
