I figured it out. A week or two ago, GNOME began mounting things that
it shouldn't (a separate bug). In this case GNOME is mounting the
device mapper device, interfering with LiveCD creator when it tries to
destroy the device.
What GNOME package would this bug be?
Probably DeviceKit-disks
DavidZ says we need to run livecd-creator wrapped from devkit-disks:
devkit-disks --inhibit -- /bin/foo
