Huge inconsistency in ACI.

I cant seem to figure something out, and i dont want to give away spoilers, but 
i think i found a problem in the book. Maybe its only in the digital edition ?
 
Its not so much a huge inconsitency, its a matter of having names mixed up for 
an entire chapter.
 
For anyone who have read it already, chapter 4 and chapter 15 dont add up 
together.
 
Did i miss something in my quick reading of the book, or are the names really 
wrong in the whole of chapter 4 ?
