Not at all. Nothing made by the hand of man is perfect, and shit happens. It's just a perfect storm of everyone expecting everyone else to catch the problem. I won't walk you through the whole thing step by step, but the key problem was when I returned the setting copy, that's when I inadvertently replaced the corrected chapter with a much earlier draft, and it wasn't until they were preparing the text for the audio book reading someone caught it.
