So sorry to hear this.  The vaccine isn't 100% effective.....was your kitten tested prior to being vaccinated?  Did your vet use a killed virus vaccine?
 
Don't second guess yourself, and don't beat yourself up.  You did what you thought was best for your kitten, and he had a good 11 months with you that he might not have had with someone else.  Big hugs to you.
