Hi Carla,


I got it at http://www.homevet.com  It's under the
link for Dr. Jeff's favorite supplements and food.
What the order form has you do is tell them your pets
name, age, breed, sex and if they are altered and any
concerns you have. When they send out your order, your
pets name is listed as a patient on the invoice. I
think this is how they are getting away with it.
