https://www.spi-inc.org/treasurer/reimbursement-form/

Then email the refund request to:
[email protected]

putting me and Michael Niedermayer in CC:, and adding the thread link from:
http://ffmpeg.org/pipermail/ffmpeg-devel/

If you can't attach receipt in the refund package, just mention it in
the email sent to the SPI treasurer.
