You are not allowed to distribute FFmpeg with libfaac and GPL enabled (because
you cannot fulfil the requirements of the GPL for libfaac), please remove it
from the executable. (You did not necessarily know that because libfaac used to
claim to be licensed under the LGPL, but unfortunately, that was never true.)

The EULA is shown when ffmpeg is installed, so it has to mention FFmpeg, must
not claim ownership for FFmpeg (it currently is claimed in point 4), must not
forbid things like copying (etc.) for FFmpeg, has to mention FFmpeg's license
(including a link to the license text) and we ask you to add a link to the
sources in the EULA. (The about window does contain a link to the sources, or
doesn't it?)
