As to the issue of trimming in QuickTime, causing the audio to get out of sync,
I think it has to do with libfaac. I wasn't able to create a properly synced
.mp4 file on my own machine, but I was successful in getting a properly synced
version on YouTube by converting the audio to PCM first, while leaving the video
alone (since it was already compressed for the web):

ffmpeg -i "input.mov" -vcodec copy -acodec pcm_s16le -ac 2 "output.mov"
