This bug I know of since ffmpeg-0.5, I have seen it at version 1.x, at 2.0
and lastly at 2.2.1.
As I am trying to run an automated encoding and distribution solution
based on ffmpeg+ffserver, this issue is a significant setback to the
reliability factor.
