#4488: Export whether or not a TrueHD stream has an ATMOS substream

This should be simple enough to detect in the parser and decoder (as far
as I'm aware, it should just be checking if the number of substreams is >
3), but I'm not sure if this makes sense to export as a profile, or
metadata, or a new member of AVCodecContext, or something else.
