So in summary,
1. Both issues (bad codec info and sample duration) is reproducible
without a reencode.
2. Doing a reencode seems to fix the bad codec information but not sample
duration.
I consider both to be defects as fragmentation should not require a re-
encode (it reduces audio quality & increases fragmentation time) and
sample duration and codec information should always be correct.
