I tried it but upon coming to question number 2: ""*2. There are a number of psycho-social and stress-related disorders that are now fairly known, sometimes resulting in burnout. (Please choose all that apply and rate the degree/extent of its effect on you)" it says choose all that apply but we can click only one, not many. Apparently, there's an error in the check box programming.
