I've had that problem, too, and it seems to be related to the clef somehow.
I've only experienced it when entering notes in a non-treble clef. Speedy
reads the cursor position as if it is in a treble staff, but then writes
the note to where it belongs in the actual staff -- thus the octave and a
sixth different when entering in a bass clef.  Try the same thing on a
different non-treble clef and you'll get a different offset.

Occurrence of this error is linked to how you enter the frame.  I don't
remember the exact details (which I once worked out, by systematic
experimentation), but if you always move to the next measure using the
arrow instead of the bracket, you can mostly avoid it -- not that that's
much of an answer, of course. Once in the measure, any arrow key will clear
the problem, but using bracket to advance to the next measure can bring it
back.

Evidently this bug only occurs under certain unusual circumstances in the
overall environment, which is why only a few of us have experienced it (all
on older Macs, I believe). But for those of us who do get it, it's very
consistent and easy to replicate. If it seems like you're experiencing it
intermittently, it's probably because you are variable in your use of the
numerous ways of Speedy navigation. But with the same keystrokes and in the
same clef, the result will be the same.

I was one of the first to report it to tech support, and at the time they
couldn't duplicate the error.  I was glad when I heard they finally
acknowledged it as a known bug.

Definitely needs to be fixed.
