I have also had this happen recently, but in 2003 under OS9.
I had 2 files open and had left the computer for a couple of hours, returned, hit cmd-Q to quit and hit return on the save dialogue assuming it would be correctly attributed. Next day I discovered one file had over-written the other (they both had completely different names) and I had 2 identical files in the same folder.
Watch out!
