There is a delightful piece from the Bach family for three players at one keyboard. It is written for two children in the middle and an adult whose hands are on the outside. I believe the score shows this with nested braces. If my memory serves me, it is called "Das Dreyblatt."

And there are some pieces PDQ Bach wrote for a chorus accompanied by "piano 5 hands" where there are 2 pianists sitting back to back, and a third who conducts the chorus and occasionally reaches over and plays a few notes on one piano or another.
Not too sure this isw a GOOD example, though. ;)
