From the Appendix of the user manual:

[Page Sizes]
This section contains the page size name, Width and Height (followed by a
semicolon), Top Margin,
Bottom Margin, Left Margin, Right Margin, and a Left Margin for
single-instrument documents.
The Top and Bottom margins are assumed to be negative; there is no need to
put in the
minus sign.
Ex. Letter = 8.5, 11; .5, .5, 1, .5, .75

I am trying to remember when was the last time I really read the manual, maybe the
explanation of pagesizes.txt has been in there forever. With every upgrade lots of things
change and I'm sure too many of them go unnoticed.
So, the next "book in bed" shall be the manual and all appendices!
