It has happened so seldom, and in some cases so long ago (it was in 
WinFin97, which haven't been using since August 2002) that I simply 
don't remember any of the conditions.

I have nothing to offer here. It hasn't happened to me lately, but 
that's because I'm not using Finale heavily these days.

Also, it's unlikely to happen to me because I almost never edit more 
than one file at a time.
