The consensus around here seems to be that charging hourly is the way to go for both services. I charge $40/hour for this, with a one-hour minimum, and I am pretty good at estimating jobs now. Average cheap-and-dirty pop lead sheets run about an hour a page, more if they need lots of detail, as for a master rhythm sheet.
Rates tend to go up near large music centres like New York, Nashville, and L.A., and are lower in the smaller centres. I am in Montreal, and charge in Canadian dollars, but my cost of living is lower, too, than a New Yorker's would be.
