It sounds to me like the lower-left-hand corner of the image is where the handle attaches. All your placement points are relative to the handle.

If you can move the handle (I can't recall having done this before) then get it to a place where you want the note to be.

If you can't move the handle (more likely) then you should enter a handle displacement value of about an inch (negative in both boxes) to make sure that the expression is positioned an inch or so closer on both axes. You will be grabbing it from then on by the handle about an inch down and to the left of where you expect it to be.

Could you possibly crop the image so that it takes up less real estate? That might make the handle closer.
